What is the Patient-Centered Medical Home?

Patient-Centered Medical Home is central to Betty Jean Kerr People’s Health Centers efforts to improve health outcomes, increase access to care and reduce health care cost. With this approach, the patient and the family work in partnership with a well-organized care team, including providers, nurses and health coaches to address all of the patient’s health care needs. The National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) is an innovative program with a set of standards that describe clear and specific criteria, give practices information about organizing care around patients, working in teams and coordinating and tracking care over time.

With NCQA Patient-Centered Medical Home standards in place, patients benefit from the facilitation of partnerships with their personal physicians, and when appropriate their families. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it. In addition, we will provide this in a culturally and linguistically appropriate manner respecting your preferences and family values. Coordination of care is assured when patients seek specialty services outside the medical home practice. When patients need to access, the care team it is by phone, email, or in-person visits to make sure patients get the care they need, when they need it.

Our goal is to expand upon strategies we have found to be beneficial to patients. With the NCQA Patient-Centered Medical Home, we will now have formal recognition of utilizing these tried and true approaches. Some of these include self-management strategies, decision-making and goal setting. When patients become active partners with the care team, patient satisfaction increases and health outcomes improve.

The Patient Centered Medical Home approach will take your experience at Betty Jean Kerr People’s Health Centers to a new level. You the patient will be at the very center of your healthcare. There will also be a stronger focus on incorporating behavioral healthcare. Patients will do more surveys that will drive quality improvement.
